REQUIRED PHYSICAL & HEALTH INFORMATION
SMA athletes must have a current physical and associated health forms on file in the FOCUS system. Students will not be allowed to participate with an expired physical or without acknowledgement of insurance.
Students (grades 6-8) planning to participate in athletics must have a completed, current physical on file with the high school Activities & Athletics Office before practice begins. Only page 5 should be uploaded into the FOCUS system.
Physical forms were emailed to participants of the June 4th Physical Night from rp.athletics@raypec.org.
Here’s a link to the physical form that your health care professional should use. Please make sure the physical is signed and dated by your healthcare professional prior to uploading.
Directions to upload a student’s physical & complete the health Form:
Log into the Focus parent portal
Select the drop-down for your student
Click on Forms Summary
Click on Physical Form
Upload page 5 of the student’s physical
Enter the date of the physical located on the bottom right corner of the physical form.
Enter the correct expiration date based on the box checked in the upper left of page 5.
The healthcare provider determines if the physical is valid for 1-year or 2-years from the date of service.
You may enter any optional comments you wish to share from the physical such as allergies, medications, etc...
Submit the completed form.
